Licensed Vocational Nurse (LVN) Visiting Home Health

Job Summary
Provide nurse visits to home health patients by performing patient care assessments, implementing RN-initiated plans of care, and delivering basic treatments. Document care consistently and communicate with patients to schedule visits while educating them on self-care during non-visit days. Demonstrate knowledge of regulatory requirements and support the Director of Nursing, Case Management, and nursing staff in delivering health care. Requires a current CA LVN license, one year of LVN and nursing experience, BLS certification, and NSO insurance. Work at least two-three days weekly with flexibility in scheduling, including one weekend and two holidays annually.
Required Qualifications
- Clinically experienced nurse
- Nursing documentation/charting skills
- Fast learner
- Great communication skills
- Highly motivated
- Positive-minded
- Visiting LVN for Home Health: 1 year
- Nursing: 1 year
- Current CA LVN License
- BLS from the American Heart Association and/or the American Red Cross
- NSO Nursing License Insurance
- Work at least two-three days each week (or ten productivity units)
- One weekend a month
- Two holidays per year (one winter and one summer)
