Licensed Vocational Nurse (LVN) Visiting Home Health

Provide nurse visits to home health patients by performing patient care assessments, implementing RN-initiated plans of care, and delivering basic treatments. Document consistently with the care plan and communicate with patients to schedule visits and educate them on non-visit day self-care. Demonstrate knowledge of regulatory requirements while supporting the Director of Nursing, Case Management, and nursing staff. Requires a current CA LVN license, BLS certification, and one year of clinical experience. Schedule includes flexibility with a minimum of two-three days per week, one weekend, and two holidays annually.
