Job Summary
Providing nursing and supportive care in the home health setting under the direction of the RN, per Interdisciplinary Plan of Care. Communicate with attending physicians, team members, and other agencies to coordinate care and resources; document all services; ensure compliance with Medicare/Medicaid home health regulations; report changes in patient condition to the RN. Requires a Texas LVN license with a minimum of 2 years of LVN experience; home health experience preferred. Part-time role with ESOP benefits and mileage reimbursement. Strong time-management skills and ability to coordinate care across the care team.
Required Qualifications
- Current Texas LVN license
- Minimum of 2 years as an LVN
- Home Health experience preferred
- Must have dependable transportation, driver’s license, and car insurance
