Job Summary
Provide nursing and supportive care per the Interdisciplinary Plan of Care under RN direction. Communicate with attending physicians, team members, and agencies to coordinate patient care and community resources. Document all services provided and demonstrate knowledge of Medicare/Medicaid regulations. Report changes in patient condition and needs to the RN. Maintain great time management skills while possessing dependable transportation, a driver's license, and car insurance. Full-time and PRN roles available in Pearland, TX, with ESOP ownership benefits, competitive pay, and 2+ years of LVN experience required.
Required Qualifications
- Current Texas LVN license
- Minimum of 2 years as a Licensed Vocational Nurse (LVN)
- Have dependable transportation, driver's license, and car insurance
Desired Qualifications
- Home Health experience preferred
