Job Summary
Part-time Licensed Vocational Nurse (LVN) needed to provide skilled nursing care at Shadelands Post Acute in Walnut Creek. Responsibilities include medication administration, wound care, monitoring vital signs, assessment and documentation of resident condition, contributing to care plans, supervising CNAs, and collaborating with an interdisciplinary team while ensuring compliance with state/federal standards. Must demonstrate strong clinical judgment, compassionate resident-centered care, and effective communication with the RN Supervisor and healthcare providers. California LVN license and BLS/CPR certification required; prior skilled nursing or post-acute care experience preferred.
