Job Summary
Deliver skilled nursing visits in the home within LVN scope under RN/clinical supervision, performing focused assessments, monitoring condition changes, and escalating promptly with clear, objective information. Support medication reconciliation and safety education, provide patient and caregiver education using teach-back, and coordinate care with the office and interdisciplinary team to ensure timely visits and adherence to orders. Document thoroughly and on time while managing wound care tasks like dressing changes and basic monitoring per competency. Work full-time Monday through Friday with some weekend availability, earning $48.08 per point with mileage reimbursement. You must hold a current Texas LVN license, a Texas driver's license, and demonstrate strong communication and documentation skills.
Required Qualifications
- Current Texas LVN license (unrestricted)
- Comfort working in a field-based workflow with appropriate escalation judgment
- Strong communication and documentation
- Reliable follow-through on orders, visit cadence, and patient goals
- Current Texas driver's license and comfort with daily travel across assigned territory
