Licensed Vocational Nurse (LVN/LPN)

Deliver skilled nursing visits in the home within LVN scope under RN/clinical supervision. Perform focused assessments, monitor for condition changes, and escalate promptly to the RN/MD/clinical team using clear, objective information. Support medication reconciliation, safety education, and wound care tasks per orders and competency. Provide patient and caregiver education aligned to the plan of care while coordinating with the office and interdisciplinary team to ensure visits occur on time. This role supports Adaptive Home Health's mission to improve patient access to high-acuity home care across Texas. You will work in a field-based setting with a per-productivity compensation model and access to charting and scheduling technology designed to reduce documentation burden and streamline visit coordination.
