Job Summary
Provide direct patient care as defined in the State's Nurse Practice Act and implement plans of care initiated by the registered nurse. Deliver accurate documentation, patient and family education, and coordinate home health services by reporting needs to the registered nurse or Clinical Supervisor. Maintain clinical records, progress reports, and case conferences, ensuring all visit notes are submitted within 48 hours. Participate in annual training sessions, short courses, and professional journal studies to advance skills. Requires current New Mexico LVN license, CPR certification, valid driver's license, and bilingual English/Spanish proficiency.
