Job Summary
Deliver skilled nursing care on a one-on-one basis in patients' private homes according to physician orders and care plans. Responsibilities include administering medications, providing rehabilitative and preventive care, observing and reporting patient changes to the RN/physician, educating family/caregivers on home-based care, and completing timely clinical documentation. Flexible scheduling available (morning, midday, night; 7-day options); new grads welcome. Must hold a California LVN license and CPR certification.
Required Qualifications

- 1 year of LVN experience preferred – but new grads welcome
