Job Summary
Provide direct patient care as a Hospice LVN under RN supervision, implementing nursing care per the plan of care, documenting services accurately, and educating patients and families. Coordinate hospice services with the interdisciplinary team, manage pain and symptoms, and use equipment effectively. Participate in quality improvement and professional development to deliver compassionate, standards-aligned hospice care in the Bay Area. Requires active California LVN license, CPR, and reliable transportation; hospice experience is preferred; weekend schedule likely.
Required Qualifications
- Active CA LVN License
- Minimum 1 year LVN experience (hospice experience preferred)
- Current CPR certification
- Valid driver’s license and insured, reliable vehicle
