Licensed Vocational Nurse, LVN - Hospice - FT

Provide direct patient care in accordance with the Nurse Practice Act, implementing individualized plans of care under Registered Nurse supervision. Deliver effective pain and symptom management while monitoring patient responses and supporting daily care needs to ensure comfort, dignity, and safety. Educate patients and families on disease progression and comfort measures, then document care accurately in the EMR. Communicate changes in patient condition to the RN and collaborate with interdisciplinary partners including social workers and chaplains for holistic support. Follow best practices, maintain equipment, and participate in quality improvement initiatives.
