Licensed Vocational Nurse (LVN) - Home Health (Visiting)

Job Summary
Provide nurse visits to home health patients by implementing RN-initiated plans of care and delivering basic treatment to ill, injured, or convalescing individuals. Conduct home assessments, communicate with patients to schedule visits, and educate them on self-care during non-visit days. Document all care consistently with the plan of care while demonstrating knowledge of regulatory requirements. Support the Director of Nursing, Case Management, and nursing staff in delivering health care services. Requires a current CA LVN license, one year of clinical experience, and BLS certification. Work 7 days a week with flexibility in scheduling, including weekends and holidays.
Required Qualifications
- 1 year LVN experience
- 1 year nursing experience
- Current CA LVN License
- BLS certification
- NSO Nursing License Insurance
- Ability to work at least two-three days each week
- Ability to work one weekend a month
- Ability to work two holidays per year
- Knowledge of regulatory requirements
