Licensed Vocational Nurse (LVN), Home Health, San Jose

Provide direct patient home care visits, performing preventative, treatment, and rehabilitative nursing procedures while implementing the plan of care. Observe home environments to recommend safety enhancements and coordinate services with patients, families, and the health care team. Take verbal orders from providers, document communications regarding condition changes, and deliver skilled interventions including injections, wound care, and vital sign monitoring. This role supports patients with chronic conditions, surgeries, or injuries to improve quality of life and reduce hospitalization risk. You will work independently in a home setting, building meaningful relationships with patients and families.
