Job Summary
Provide skilled nursing care to clients in their residences under the direction of a registered nurse and physician's plan of care. Promote client health through teaching, rehabilitative measures, and self-care instruction while preparing clinical progress notes and adhering to established care plans. Assist physicians and RNs in specialized procedures, ensure aseptic technique during equipment preparation, and maintain continuity by communicating with interdisciplinary professionals. Participate in on-call duties, instruct and evaluate home health aides every fourteen days, and attend client conferences or quality improvement reviews as required.
Required Qualifications
- Successful completion of a certified licensed practical/vocational nursing program
- Licensed to practice as a practical/vocational nurse in the state
- Minimum of six (6) months to one (1) year of clinical experience
- Reliable transportation
- Valid and current auto liability insurance
- Must be a licensed driver with an automobile that is insured in accordance with state/or Company requirements and is in good working order
- Must hold a current and valid CPR certification
Desired Qualifications
- Six (6) months to one (1) year of previous home health or hospice experience
