Job Summary
Administer nursing care under the direction of a registered nurse for clients in their place of residence, coordinating with the interdisciplinary team, clients, and families to implement the Plan of Care. Identify physical, psychosocial, and environmental needs through documentation and case conferences, while ensuring infection control measures protect both client and staff. Monitor assigned cases for third-party payer compliance, complete timely documentation, and participate in regular Case Conference/Team Meetings. Provide effective communication to clients, families, and healthcare professionals, and maintain competency through mandatory continuing education and in-service programs. Requires one year of clinical LVN/LPN experience, current license, CPR certificate, and ability to travel locally for flexible schedules.
Required Qualifications
- 1-year experience as an LVN/LPN in a clinical care setting
- Preferred home health experience
- Nursing skills as defined as generally accepted standards of practice
- Excellent interpersonal skills and ability to communicate effectively
- Demonstrates proven decision-making skills
- Hepatitis profile
- Must read, write and comprehend English
- Completion of an accredited Licensed Vocational/Licensed Practical Nursing program
- The current license is in good standing to practice as an LVN/LPN in any/all applicable states
- Current CPR certificate
- Current driver's license in good standing
- On-going employment as an LVN/LPN requires maintaining a valid LVN/LPN license in the state where the nurse practices and a valid CPR certificate at all times
- Prolonged standing/walking is required
- The ability to lift/transfer up to 50 lbs
- Ability to meet deadlines and patient needs, make quick decisions, and allocate resources under stressful conditions
- Meet patient/family individualized psychosocial needs
- Requires hand-eye coordination and manual dexterity
- Ability to travel locally and work a flexible schedule
- Reliable transportation and auto liability insurance
