Licensed Vocational Nurse (LVN) Home Health

Implement the plan of care initiated by the Registered Nurse and provide accurate, timely documentation consistent with that plan. Assess and educate patients and caregivers regarding diagnosis, care plans, and treatments while using equipment and supplies effectively. Participate in coordination of home health services by reporting additional needs for HHA, PT, OT, ST, or MSW to the RN or Clinical Supervisor. Complete online charting in Kinnser using company-provided devices and follow all company policies and procedures. Maintain an active California LVN license and valid driver's license with at least one year of nursing experience.
