Job Summary
Provide direct nursing care to residents by administering medications, monitoring vital signs, and assessing condition changes. Assist with individualized care plans, activities of daily living, and emergency response while ensuring accurate documentation. Coordinate care with physicians, therapists, and other healthcare professionals under RN direction. Adhere to all facility policies and regulations governing nursing care. Requires valid California LVN license, one year of skilled nursing experience, and proficiency in med pass and Point Click Care. PM shift (3PM-11PM) available.
Required Qualifications
- Valid and current LVN license in the state of California
- Graduate of an accredited nursing program
- Minimum of 1 year experience in a skilled nursing facility, acute care setting, or in-patient rehabilitation or in-patient behavioral health setting
- Strong clinical skills, including assessment, planning, and implementation of nursing care
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Strong critical thinking and problem-solving skills
- Experience with med pass
- Experience with Point Click Care
