Job Summary
Licensed Vocational Nurse (LVN) provides care coordination support for CalAIM Enhanced Care Management (ECM) within Hiring County, performing member assessments, health education, medication reconciliation under RN supervision, and field/home visits. Responsibilities include clinical outreach, identifying barriers to care, coordinating with interdisciplinary teams, documentation in electronic health records, adherence to Medi-Cal/ECM requirements, and ensuring member safety. Requires LVN licensure in California, valid driver’s license, ability to travel up to ~60-70% within the county, hybrid remote/field work in Hiring County, and strong communication and organization skills. Bilingual abilities and experience with underserved populations are preferred. Compensation is $30.00-$35.00 per hour (hybrid schedule; some field work and remote tasks).
Required Qualifications
- Current and unrestricted California LVN license
- Minimum one (1) year of clinical healthcare experience
- Ability to travel 60-70% within Hiring County
- Valid California Driver's License (Class C)
- Reliable transportation and automobile insurance
- Proficiency with electronic health records (EHR) and Microsoft Office
- Successful completion of background checks and MVR
- Able to work in a hybrid remote/field environment
- Ability to pass skills assessment (Testlify)
- Must reside in or near Hiring County
- Must be able to work evenings/weekends as needed
