Job Summary
Admitting, transferring, and discharging residents while conducting daily rounds to monitor their physical and emotional status. Administer medications and oversee care provided by unlicensed staff under the direction of a registered nurse. Maintain confidentiality of resident information and promote a culture of safety within the facility. Ensure residents and families receive high-quality care in a compassionate, family-oriented environment. This role requires current LVN/LPN licensure, CPR/BLS certification, and strong communication skills. Experience in a Skilled Nursing Facility or memory care is preferred.
Required Qualifications
- LVN / LPN with current licensure to practice in the state
- CPR/BLS Card
- Strong communication and organizational skills
- Basic knowledge of computer use or willingness to learn
Desired Qualifications
- Experience in a Skilled Nursing Facility (SNF) environment
- Nursing experience providing direct care to multiple residents
- Behavioral care / memory care experience
- Experience with PointClickCare (PCC)
