Job Summary
Receive case referrals, review patient information to determine home care needs, and assign appropriate clinicians. Evaluate services provided by clinicians, discuss impressions, and guide staff to promote effective quality home care delivery. Review clinical diagnoses, medications, and procedures; assist in establishing therapeutic goals and developing care plans. Attend case conferences, conduct quarterly record reviews, and communicate findings to the Clinical Director. Assist in screening new personnel, orientation, and inservice education programs while helping formulate and evaluate policies and strategic goals.
Required Qualifications
- Licensed vocational nurse with current licensure to practice professional nursing in the State
- Two years case management experience within the past five years in a home health agency
- Knowledge and expertise in clinical decision-making and Quality Management review and evaluation
- Complies and maintains current CPR certification
- Demonstrates excellent observation, verbal and written communication skills
Desired Qualifications
- Nursing degree is preferred
