Job Summary
Deliver skilled nursing visits in the home within LVN scope under RN/clinical supervision, performing focused assessments, monitoring for condition changes, and escalating promptly to the RN/MD/clinical team. Support medication reconciliation and safety education, provide patient and caregiver education using teach-back methods, and coordinate care with the office and interdisciplinary team to ensure visits occur on time. Manage wound care and other skilled tasks per orders and competency while maintaining clear, objective documentation. Adaptive Home Health is building a higher-acuity, patient-centered home health model across Texas, utilizing charting and scheduling technology designed to reduce documentation burden and streamline visit coordination.
Required Qualifications
- Current Texas LVN license (unrestricted)
- Comfort working in a field-based workflow with appropriate escalation judgment
- Strong communication and documentation
- Reliable follow-through on orders, visit cadence, and patient goals
- Current Texas driver's license and comfort with daily travel across assigned territory
