Job Summary
Provide comprehensive care in patients' homes, including administering medications, performing wound care and infusions, and delivering disease management education. Complete skilled documentation through Homecare Homebase and serve as the patient's advocate by maintaining communication with physicians, caregivers, and facility management. Participate in regular case conferences and in-services while managing a caseload of approximately six patients daily from Monday through Friday. This role requires a current Texas LPN license with three years of clinical experience and proficiency in healthcare software.
Required Qualifications
- LPN license
- Current Texas LPN license
- Three years of clinical nursing experience
- Current Texas driver's license
- CPR Required
- BLS Certification
- Competency to carry out any skilled procedure that the physician orders that is within the scope of the Nurse Practice Act
- Comfortable treating wounds
- Administer medications and treatments as prescribed by physician
- Provide direct patient care including infusion therapy, wound care, teaching, disease management
- Service ~ 6 patients a day M-F
- Strong organization skills and time management
- Able to multitask and accomplish and meet timelines
- Professional appearance
- Effective communication skills
- Experience with healthcare software
- Monday to Friday schedule
- Weekends availability
