Job Summary
Conduct home visiting nursing care for patients in the San Gabriel Valley and surrounding areas as a per diem field-based LVN. Manage time efficiently while working independently, utilizing EMR systems such as MyUnity where applicable. Maintain current CPR, physical exam, and TB test status while ensuring all immunization records are up to date. This role offers flexible scheduling and the opportunity to grow with an established agency committed to providing high-quality home and hospice care.
Required Qualifications
- Active California LVN License
- Current CPR
- Driver's License
- Car Insurance
- Social Security Number
- Current Physical Exam and TB Test
- Immunization Records (Flu, Hep B, TDAP, MMR and Varicella)
- Minimum of one year of nursing experience, including Home Health or similar environment experience is preferred
Desired Qualifications
- Strong clinical, organizational, and communication skills
- Ability to work independently and manage time efficiently
- Familiarity with EMR systems (MyUnity experience is a plus)
- Being bilingual is a plus (Spanish, Vietnamese, Chinese)
- This opportunity is also open to a clinically experienced LVN who is a self-starter in home health
