Provide direct patient care as an LPN/LVN under physician orders, monitoring vital signs and patient responses, and reporting findings to Registered Nurses to execute personalized care plans. Build meaningful relationships with patients to support physical, mental, and emotional recovery, monitor and document patient progress, and collaborate with the care team to advance rehabilitation goals.
