Job Summary
Conduct rounds to observe, assess, document, and report resident condition changes. Administer medications as prescribed and ensure implementation of care plans. Perform narcotic reconciliation, initiate incident reports, and manage resident discharges per facility policy. Monitor nursing aides and communicate status updates to resident point of contact. Must complete nursing assessments before physician calls. Requires active Texas LVN license and experience in skilled nursing facilities.
Required Qualifications
- Has completed an LVN program at an accredited vocational school or college.
- Must be an active Licensed Vocational Nurse in the state of Texas in good standing.
- Must be able to lift 1-25 lbs
