Job Summary
Conduct rounds to observe, assess, document, and report resident conditions or changes. Administer medications as prescribed and ensure implementation of care plans. Perform narcotic reconciliation with incoming and outgoing shifts and oversee aides. Identify significant changes in resident conditions and take necessary action per facility policies. Initiate and complete incident and accident reports, discharge residents, and communicate with resident point of contacts. Complete nursing assessments for physician calls prior to contacting physicians. Assist in ensuring quality care that promotes the highest practicable physical, mental, and psychological well-being of residents.
Required Qualifications
- Has completed an LVN program at an accredited vocational school or college.
- Must be an active Licensed Vocational Nurse in the state of Texas in good standing.
- Must be able to lift 1-25 lbs

Desired Qualifications
- Experience working in a skilled nursing facility is preferred.
