Job Summary
Conduct rounds to observe, assess, document, and report resident conditions while administering medications as prescribed. Ensure implementation of care plans, perform narcotic reconciliation, and identify significant changes in resident status to take necessary action per facility policies. Oversee and monitor nursing aides, initiate incident and accident reports, and discharge residents according to established procedures. Complete nursing assessments for physician calls prior to contacting providers and communicate status updates to resident point of contact. Requires active Texas LVN license, skilled nursing facility experience preferred, and ability to lift up to 50 lbs and perform various physical tasks.
Required Qualifications
- Has completed an LVN program at an accredited vocational school or college.
- Must be an active Licensed Vocational Nurse in the state of Texas in good standing.
- Must be able to lift 1-25 lbs
- Must be able to lift 25-50 lbs
- Must be able to carry 1-25 lbs
- Must be able to carry 25-50 lbs
- Must be able to push/pull 1-25 lbs
- Must be able to push/pull 25-50 lbs
