Job Summary
Conduct daily rounds to observe, assess, document, and report resident conditions while administering medications as prescribed. Ensure implementation of care plans, perform narcotic reconciliation, and oversee nursing aides. Identify significant resident changes, initiate incident reports, and manage discharges per facility policy. Communicate status updates to resident contacts and complete nursing assessments before physician calls. Maintain quality care standards through adherence to individual resident plans and facility procedures.
Required Qualifications
- Has completed an LVN program at an accredited vocational school or college.
- Must be an active Licensed Vocational Nurse in the state of Texas in good standing.
- Must be able to lift 1-25 lbs
- Must be able to lift 25-50 lbs
- Must be able to lift 50+ lbs
- Must be able to carry 1-25 lbs
- Must be able to carry 25-50 lbs

Desired Qualifications
- Experience working in a skilled nursing facility is preferred.
