Job Summary
Conduct rounds to observe, assess, document, and report resident conditions, ensuring care plans are implemented as directed. Administer medications per prescription and perform narcotic reconciliation across shifts. Identify significant resident changes to initiate necessary actions and Incident/Accident Reports per facility policy. Oversee aides, discharge residents, and complete Nursing Assessments before physician calls. Communicate status updates to resident point contacts. Requires active Texas LVN license with skilled nursing facility experience preferred.
Required Qualifications
- Has completed an LVN program at an accredited vocational school or college.
- Must be an active Licensed Vocational Nurse in the state of Texas in good standing.
- Must be able to lift 1-25 lbs

Desired Qualifications
