Job Summary
Conduct rounds to observe, assess, document, and report resident conditions while ensuring care plan implementation. Administer medications as prescribed and perform narcotic reconciliation across shifts. Monitor nursing aides, initiate incident reports, and manage resident discharges per facility policy. Communicate status updates to resident contacts and complete nursing assessments before physician calls. Lift and transfer residents up to 50+ lbs, stand, walk, and perform various physical tasks as required. Requires active Texas LVN licensure with skilled nursing facility experience preferred.
Required Qualifications
- Has completed an LVN program at an accredited vocational school or college.
- Must be an active Licensed Vocational Nurse in the state of Texas in good standing.
- Must be able to lift 1-25 lbs
- Must be able to lift 25-50 lbs
- Must be able to carry 1-25 lbs
- Must be able to carry 25-50 lbs
- Must be able to push/pull 1-25 lbs
- Must be able to push/pull 25-50 lbs

Desired Qualifications
- Experience working in a skilled nursing facility is preferred.
