Job Summary
Licensed Vocational Nurse (LVN) Charge Nurse in San Antonio, TX who oversees nursing care for residents in a skilled nursing facility. Responsibilities include observing and assessing resident conditions during rounds, ensuring implementation of care plans, administering medications, conducting rounds with physicians and documenting visits, performing narcotic reconciliation, identifying changes in condition and taking action per policy, overseeing aides, initiating and completing incident/accident reports, discharging residents per policy, and performing nursing assessments before physician calls. Requires active Texas LVN license and LVN program completion; prior SNF experience preferred; strong communication with residents’ point of contact and collaboration with nursing leadership.
Required Qualifications
- Active Licensed Vocational Nurse in Texas
- Has completed an LVN program at an accredited vocational school or college
- Experience working in a skilled nursing facility (preferred)
- Must complete Nursing Assessment for Physician Calls prior to calling physician on any change in resident condition
