Job Summary
Conduct direct social work services for senior adults and caregivers via telehealth or home visits, performing comprehensive assessments to develop care plans. Collaborate with families, clinicians, and referring partners to address unmet needs, promote STARS program offerings, and advocate for client goals while managing crisis situations. Maintain constant communication to help patients maintain independence and improve quality of life. Requires a Master's degree, current LSW license in Illinois or Nevada, and experience with geriatric populations. Flexible full-time or part-time schedule with options for evenings and weekends.
Required Qualifications
- Provide mental or clinical social work services both telephonically and in the home of clients
- Carry out comprehensive social work assessments by phone or in-person via home visits to develop a plan of care for the client
- Work directly with patients and their families to address unmet needs and provide the necessary social work services to help patients and support achieve their care goals
- Maintain relationships with referring partners through consistent and timely communication
- Promote the STARS program and the STARS CRDC Resource Centers program offerings by helping clients to connect with programs that address their needs
- Evaluate patients (existing, new, and prospective) based on their needs, limitations, and desires
- Address concerns and goals while maintaining constant communication with the patient
- Build rapport with patients and their families
- Respond to crisis situations that affect vulnerable or mentally unstable seniors
- Advocate and raise awareness on behalf of clients and the needs of the community
- Proficient in Microsoft Office Suite

- Self-directed and able to work without supervision
- Extensive background in Rehab Facilities
- Extensive background in Hospital
- Extensive background in Depression
- Extensive background in Trauma
- Master's degree in social work, psychology, sociology, healthcare, or another related field
- Valid and current LSW (Illinois) or (Nevada)
- 2-5 years of professional social work experience
- Experience in Case Management
- Working with geriatric patients
