Job Summary
Conduct psychosocial assessments and complete social histories for each resident to identify emotional and psychological needs. Maintain accurate documentation of care plans, Minimum Data Sets, and progress notes while supporting resident self-determination and documenting service refusals. Participate in developing interdisciplinary care plans that address psychosocial issues and goals. Provide access to services aligned with the NASW Code of Ethics and federal regulations. Requires a licensed Bachelor's degree in Social Work and one year of supervised healthcare experience.
Required Qualifications
- Certified, licensed, or registered in the state of practice
- Bachelor's Degree in Social Work
- Bachelor's Degree in Human Services field, including but not limited to Sociology, Special Education, Rehabilitation Counseling and Psychology from an accredited school of social work
- One year of supervised social work experience in a healthcare setting working directly with geriatric individuals
Desired Qualifications
- Minimum of 2 years of nursing experience in a Skilled Nursing Facility
