Job Summary
Coordinate telehealth provider services by facilitating screening assessments, managing referrals, and maintaining family contact under limited supervision. Lead the clinical team in delivering comprehensive care while ensuring thorough and efficient collaboration with medical professionals. Maintain telehealth equipment and perform required physical tasks including lifting patients and operating in various indoor and outdoor environments. Work rotating shifts and overtime as needed to support the Access Bridge Care program at MUSC Community Physicians.
Required Qualifications
- Bachelor's degree in social work or social welfare program accredited by the Council on Social Work Education, or related degree
- MSW licensure by the South Carolina Board of Social Work Examiners
- One year post graduate professional social work experience preferred

- Ability to lower objects to 35lbs from height of 36 inches, unassisted
- Ability to push/pull objects to 100lbs, unassisted
- Ability to push/pull patients to 300lbs(+/-) in wheelchairs, etc., unassisted
- Ability to maintain 20/40 vision, corrected
- Ability to see & recognize objects close at hand
- Ability to see & recognize objects at a distance
- Ability to match or discriminate between colors
- Ability to determine distance/relationship between objects; depth perception
- Good peripheral vision capabilities
- Ability to maintain hearing acuity, with correction
- Ability to hear & understand whispered conversations at a distance of 3 feet
- Ability to perform gross motor functions with frequent fine motor movements
- Ability to be qualified physically (by medical personnel) for respirator use, initially & annually
- Ability to deal effectively with stressful situations
- Ability to work rotating shifts
- Ability to work overtime as required
- Ability to learn & use new processes, tools, equipment as required
