Licensed Social Worker - PRN

Job Summary
Provide psychosocial assessments and counseling to patients and families dealing with terminal illness; assist with advance directives, funeral arrangements, and navigating legal/financial decisions; coordinate with community resources and support organizations; educate staff and families about cultural and psychosocial needs; document interventions and serve as liaison between families and care teams; participate in interdisciplinary team meetings to develop individualized care plans; provide crisis intervention and on-call support as needed.
Required Qualifications
- Bachelor�s degree in Social Work (BSW) required
- Master�s degree (MSW) or LCSW strongly preferred
- At least 1 year of experience in social work, preferably in hospice, home health, or a community setting
- Knowledge of end-of-life care, grief support, and cultural sensitivity
- Strong written and verbal communication skills
- Self-motivated, organized, and able to work independently
- Current Basic Life Support (CPR) certification
- Valid driver�s license, auto insurance, and reliable vehicle required
- Pre-employment drug screening is required, including testing for marijuana
