Licensed Social Worker (MSW) in Idaho: Performs psychosocial assessments of patients with terminal illness and their families/caregivers, documents findings to the interdisciplinary team, conducts social evaluations, plans interventions, counsels on stress and coping, maintains clinical records, provides information and referrals, serves as liaison with community agencies, develops contracts for resources, supports volunteer services, participates in the patient care plan and regular interdisciplinary meetings, adheres to care procedures and standards, helps understand social and emotional factors related to health and death, and participates in quality assessment and performance improvement activities.
