Initiate direct patient care services, family and patient counseling, discharge planning, crisis intervention, and consultation. Coordinate referrals and promote educational programs while making independent decisions in collaboration with team members. Provide direct patient care to all ages through the continuum of care. Maintain access to confidential employer, patient, and financial information. Requires a Master's degree in social work, current Ohio Social Work license, and one year of social work experience. Prior home care or discharge planning experience preferred. Full-time day shift Monday through Friday in Waverly, Ohio.
