Job Summary
Conduct pre-opening licensure surveys and policy reviews for Licensed Home Care Services Agencies across New York City's five boroughs. Review agency documents, personnel files, and client records to ensure compliance with Article 36 Title 10 regulations. Write letters to agencies regarding non-compliance or recommend licensure decisions to the Department of Health. Prepare for and execute on-site visits, including reviewing QI meeting minutes and admission packets. Submit Statements of Deficiencies via the ASPEN computer program when necessary. Assist with scheduling site visits and track work process aspects as directed. Maintain flexibility for frequent travel and field-based environments while collaborating with co-reviewers and the Director.
Required Qualifications
- Master's degree in Social Work
- Licensed in New York State (LMSW/LCSW)
- Flexibility to travel at times for several hours
- With satisfactory evaluation, committed to remain in position for at least one year
Desired Qualifications
- Two years of experience in social work preferred
- Experience in home care
- Demonstrates logical reasoning & critical thinking skills
- Detail oriented, highly organized work skills
- Demonstrates a mature, professional, respectful, demeanor and style
- Excellent verbal and written communication skills
- Computer skills including proficiency in Microsoft Word and Microsoft Excel
- Quality improvement orientation
