Job Summary
Provide compassionate social work services to residents and families in long-term care settings. Develop social histories and assessments; create individualized care plans; participate in interdisciplinary care plan conferences; provide individual and group support during crises; assist with new resident orientation; coordinate discharge planning; facilitate staff in-service training; build relationships with external partners (hospitals, doctors, churches, community agencies) to support referrals and ongoing care.
Required Qualifications
- Bachelor’s Degree in Social Work (BSW) or certification as a Social Worker Designee with relevant secondary education
