Job Summary
Provide compassionate social work support to residents and families in a long-term care setting. Develop detailed social histories and assessments, create individualized care plans, participate in interdisciplinary care plan conferences, provide individual and group support during crises or transitions, assist with resident orientation and discharge planning, and facilitate staff training. Build relationships with external partners to support referrals and ongoing care. Must hold a BSW or equivalent Social Worker Designee certification and demonstrate effective communication, organization, and a compassionate approach to holistic resident well-being.

- Bachelor’s Degree in Social Work (BSW) or certification as a Social Worker Designee
