Licensed Social Worker - LMSW or LBSW

Provide social work support to residents and families in a long-term care setting by developing detailed social histories and assessments, creating individualized care plans to support resident goals and emotional well-being, and participating in interdisciplinary care plan conferences. The role includes providing individual and group support during crises or transitions, assisting with new resident orientation, coordinating discharge planning, and facilitating staff training related to social services. Build strong relationships with external partners (hospitals, doctors, churches, community agencies) to support referrals and ongoing care. A Bachelor’s in Social Work (BSW) or certification as a Social Worker Designee is required, with experience in long-term care, and strong communication and interpersonal skills are essential.
