Job Summary
Licensed Social Worker role focused on supporting residents and families in a long-term care setting. Responsibilities include developing detailed social histories and assessments, creating individualized care plans to support residents’ goals and emotional well-being, participating in interdisciplinary care plan conferences, providing individual and group support during crises or transitions, assisting with new resident orientation, coordinating discharge planning, facilitating staff in-service trainings, and building relationships with hospitals, doctors, churches, and community agencies to support referrals and ongoing care.
Required Qualifications
- Bachelor’s Degree in Social Work (BSW) or certification as Social Worker Designee with relevant secondary education
- Demonstrates effective communication and interpersonal skills
- Experience working in long-term care settings
- Compassionate, organized, and committed to holistic well-being of residents
