Job Summary
Conduct psychosocial assessments of patients and caregivers while providing counseling on emotional stress, coping, grief, and advance care planning. Assist with advance directives, funeral arrangements, and navigating legal/financial decisions by identifying and coordinating with community resources. Serve as a liaison between families and care teams to ensure coordinated, holistic care, including participation in interdisciplinary meetings and quality improvement initiatives. Provide crisis intervention for safety concerns such as neglect, abuse, or self-harm. Document patient conditions, interventions, and support provided. Participate in on-call support as needed. Requires BSW, MSW or LCSW, one year experience, CPR certification, and valid driver's license.
Required Qualifications
- Bachelor's degree in Social Work (BSW)
- At least 1 year of experience in social work, preferably in hospice, home health, or a community setting
- Knowledge of end-of-life care, grief support, and cultural sensitivity
- Strong written and verbal communication skills
- Self-motivated, organized, and able to work independently
- Current Basic Life Support (CPR) certification
- Valid driver's license, auto insurance, and reliable vehicle
- Pre-employment drug screening
