Job Summary
Social Worker for WesleyLife Hospice/Palliative Care visiting patients in their homes to assess psychosocial status, safety, and caregiver needs; communicate findings to care teams to update care plans; conduct social evaluations and plan interventions; provide counseling to patients and families; assist with information/referrals for financial, practical, and environmental needs; serve as liaison with patients, families, care providers, and community agencies; participate in caregiver, placement, and discharge planning as needed. LISW preferred and LMSW encouraged; pre-hire health assessment and drug screen required; mileage reimbursement offered; daily pay available; employer is an equal opportunity employer.
Required Qualifications
- LISW preferred
