Job Summary
Provide clinical consultation to supervisory and case management staff and link families with resources for housing, education, medical, and other specialized services. Collaborate with Parenting, Nutrition, and Exercise Partners to manage incoming parenting referrals, coordinate group schedules, co-facilitate parenting and adolescent groups, and create a community speakers schedule. Develop and maintain positive partnerships with community stakeholders, serve as a liaison to public and charter schools, and engage in outreach events. Track supplies, give-a-ways, and gifts provided to enrolled families. Attend community meetings. Ideal candidates must reside in Philadelphia, PA, or surrounding Tri-State areas. Master of Social Work with LCSW or LSW license and two years of clinical experience preferred.
Required Qualifications
- Master of Social Work
- Licensed Clinical Social Worker (LCSW) or Licensed Social Worker (LSW)
- Two years of clinical experience
- Three to five years of social work or case management experience in a child welfare agency, prevention services, or behavioral health agency
- Knowledge of office operations
- Proficient PC skills, including Microsoft Office software, and Electronic Medical Records Systems
- Interpersonal skills sufficient to interact with sensitivity with patients, families, office staff, and external parties
- Reside in Philadelphia, PA, or surrounding Tri-State areas
- Willing to relocate to the Philadelphia, PA area
