Job Summary
Licensed Social Worker Director responsible for planning, organizing, implementing, evaluating and directing the Social Service Department; provide medically related social work services to achieve the highest practicable physical, mental and psychosocial well-being; duties include Resident/Family/Physician Intervention, Psychosocial Services, Vision/Hearing/Dental Services, Assessments & Care Planning, Discharge Planning; requires an LSW in Texas, a 4-year degree, and at least 2 years of skilled nursing facility or hospital experience; full-time role in Houston, TX with benefits including Health/Dental/Vision, PTO, tuition reimbursement, 401(k), and EAP.
Required Qualifications
- 4 year Degree
- Licensed Social Worker in the State of Texas
- Social Worker License
- At least 2 years experience in a Skilled Nursing Facility or Hospital
