Licensed Social Worker

Oversee medically related social service needs by assessing residents to support their physical, mental, and psychological requirements. Maintain contact with families to report health changes and discharge planning progress. Assist with financial and legal matters, including applying for Medicaid, Medicare, and advance directives. Act as a liaison with community agencies to coordinate referrals and secure adaptive equipment. Plan room visits for isolated residents and organize activities to promote social interaction and intellectual stimulation. Manage admissions, discharge planning, and counseling while ensuring documentation of all resident rights and care plans.
