Job Summary
Conduct psychosocial assessments and complete Minimum Data Sets for each resident to identify emotional and psychological needs. Maintain accurate documentation of care plans, discharge planning, and progress notes while supporting resident self-determination and rights. Participate in interdisciplinary care planning and provide therapeutic interventions for transitions to long-term care facilities. Coordinate referrals for home care services and educate families on placement issues. Prepare for inspection surveys and address immediate corrections noted by surveyors.
Required Qualifications
- Active Social Worker's license to practice
- Certified, licensed, or registered in the state of practice
- Bachelor's Degree in Social Work
- Bachelor's Degree in Human Services field, including but not limited to Sociology, Special Education, Rehabilitation Counseling and Psychology from an accredited school of social work
- One year of supervised social work experience in a healthcare setting working directly with geriatric individuals
Desired Qualifications
- Minimum of 2 years of nursing experience in a Skilled Nursing Facility
