Licensed Social Worker Ambulatory Wailuku - Full-Time Day Shift

Conduct psychosocial assessments, develop service plans, and coordinate care for patients and families while providing consultation to healthcare staff. Manage case interventions, resource referrals, and advocacy services to prevent dysfunction and enhance personal functioning. Assist with program administration, policy implementation, quality improvement processes, and clinical record audits. Perform research on social systems and participate in departmental functions, staff meetings, and new personnel orientation. Maintain educational records to demonstrate competency and ensure documentation adheres to Federal and State regulations.
