Licensed Social Worker Ambulatory Wailuku - Full-Time Day Shift

Conduct psychosocial assessments, diagnostic impressions, and case coordination for patients and families. Develop and implement social work service plans while providing consultation to staff and healthcare providers. Manage resource identification, referrals, advocacy, and mediation services to enhance personal and social functioning. Coordinate patient placement, perform clinical record audits, and implement quality improvement processes. Assist with departmental annual work plans, participate in performance improvement, and conduct research on social systems and policies. Maintain educational records and ensure compliance with Federal and State regulations.
